Common misconceptions about neuroplasticity
Neuroplasticity is the activity-dependent remodelling of synaptic strength, axonal wiring and network excitability. This page separates what has been measured in animals and in vitro from what remains unproven in humans.
Neuroplasticity is the umbrella term for activity-dependent changes in the nervous system: synaptic potentiation and depression, dendritic and axonal sprouting, altered myelination, and shifts in network excitability. Synonyms in the literature include neural plasticity, neuroadaptation and, in the rehabilitation setting, experience-dependent plasticity. It is not a single molecule and has no sequence, formula or molecular weight; it is a class of processes defined by measurement, and the identifiers that matter are the assay and the model. In the studies below those identifiers are specific: intermittent theta burst stimulation (iTBS) to the left dorsolateral prefrontal cortex in treatment-resistant depression [2], photothrombotic ischemic stroke in mice [5], cocaine self-administration in mice [3], post-stroke aphasia in humans [4][7], and lumbar spinal stenosis with endoscopic decompression [6].
The pathway families that recur across this work are glutamatergic synaptic signalling, neurotrophic and cytokine signalling, and the gut-brain axis; the receptor and transporter families named in the evidence are not uniform, and no single receptor is the target of "neuroplasticity" as such.
Research Studies
Intermittent theta burst stimulation and peripheral biomarkers in treatment-resistant depression
Treatment-resistant depression is a substantial clinical problem: a large proportion of people with major depressive disorder do not respond adequately to conventional antidepressants. iTBS is a non-invasive neuromodulation technique directed at the left dorsolateral prefrontal cortex and has been used as a treatment option in this population, but the biological basis of who responds remains poorly understood. A single-centre, prospective, investigator-initiated study is examining the effects of iTBS on clinical symptoms, executive function, and peripheral inflammatory and neuroprotective biomarkers [2]. The design treats the peripheral marker panel as the explanatory variable rather than assuming that stimulation produces uniform plasticity across patients.
Incretin therapy and exercise in type 2 diabetes
A randomized study in adults with type 2 diabetes who are initiating incretin-mimetic therapy is testing whether adding a structured exercise programme produces additional benefit over the drug alone. Participants are randomized to incretin-mimetic therapy alone or incretin-mimetic therapy plus structured exercise for 24 weeks; those assigned to drug alone are offered the same exercise programme after the 24-week period. Brain health is assessed through cognitive performance, mental health, and brain-mediated measures [1]. The comparison is between two active conditions, which is the relevant design if the question is whether exercise adds to, rather than substitutes for, pharmacological effect.
Gut Proteobacteria glycine metabolism and cocaine self-administration in mice
In mice, cocaine exposure altered gut microbiome composition, increasing the abundance of Proteobacteria. That shift depleted host glycine, changed cocaine-induced transcriptional responses in the nucleus accumbens, and facilitated behavioural sensitization and reinstatement of cocaine self-administration [3]. This is an animal model. The chain from microbial composition to host amino acid availability to transcriptional change in a reward-related nucleus is the mechanistic content; the behavioural endpoints are mouse endpoints.
Cerebrolysin as an adjunct to speech-language therapy in post-stroke Wernicke's aphasia
A case report describes a 38-year-old female lawyer who developed acute Wernicke's aphasia and right-sided hemiparesis after ischemic stroke. Despite timely thrombolysis and mechanical thrombectomy she had significant neurological deficits; early multidisciplinary rehabilitation was combined with a 10-day course of Cerebrolysin [4]. A single case cannot establish efficacy, and the report is included here because it describes the adjunctive protocol and the clinical context, not because it demonstrates a treatment effect.
Cerebrolysin added to standard treatment in non-fluent post-stroke aphasia
A randomized, placebo-controlled, double-blinded, single-centre study is measuring change in language ability when Cerebrolysin is added to standard treatment for non-fluent aphasia after stroke. Patients are divided into an experimental group receiving speech therapy plus Cerebrolysin and a control group receiving speech therapy alone, with PK-WAB scores compared before and after administration [7]. The registry entry notes that 15–42% of patients in the subacute phase of stroke and 25–50% of patients with chronic stroke show symptoms, which frames the size of the population in question. This is the controlled design that the case report above cannot provide.
Transcriptional factor expression and corticospinal axon sprouting after stroke in mice
Spontaneous recovery after ischemic stroke is limited, and age-related decline in neuroplasticity is a major proposed contributor. In mice, ectopic expression of a transcriptional factor cocktail (Oct4, Sox2 and Klf4, referred to as OSKTFs) reset developmental decline of epigenetic signatures in adult corticospinal neurons without altering their spinal projection patterns or their function in skilled locomotion. Corticospinal expression of OSKTFs had moderate effects on collateral sprouting of corticospinal tract axons and on recovery of skilled motor function after photothrombotic stroke; the effect was greater when combined with task-dependent rehabilitation [5]. The finding is a mouse finding, and the combination arm is the part that speaks to rehabilitation biology rather than to the transcription factors alone.
Cytokines and neuroplasticity modulators in lumbar spinal stenosis and endoscopic decompression
Lumbar spinal stenosis is a degenerative musculoskeletal disorder in which narrowing of the spinal canal produces pain and disability. Hypertrophy of the ligamentum flavum is a primary contributor, alongside facet joint degeneration, disc herniation and ligament ossification, and the pathophysiology is incompletely understood. Histological changes in hypertrophied ligamentum flavum include fibrocartilaginous metaplasia, type II collagen proliferation, ossification, calcium crystal deposition, and degeneration of elastic and collagen fibres [6]. A study is examining cytokines, neuroplasticity modulators and biomarkers in this condition and in the context of endoscopic decompression [6].
Psilocybin, psilocybin-assisted CBT and neuroplasticity markers in major depressive disorder
A randomized controlled trial is evaluating psilocybin alone, cognitive behavioural therapy alone, and psilocybin-assisted CBT in major depressive disorder. Participants continue their routine depression medications and are assessed for changes in depression scores, biochemical markers, inflammatory markers, neuroplasticity markers and brain activity patterns using validated tools [8]. The three-arm structure allows the pharmacological and psychotherapeutic components to be separated, which is the design question that dominates this literature.

Two further lines of evidence sit behind this page without being its subject. A review of the skin-brain axis describes a bidirectional neuroimmune network linking cutaneous inflammation with central neuroinflammation, stress signalling and behavioural disturbance, and evaluates pharmacological strategies targeting that communication [9]. A review of exercise-induced neuroadaptation describes exercise as a biologically active stimulus acting on stress-responsive neural systems across molecular, autonomic, neurochemical, neuroimmune and network levels, drawing on human intervention studies and neuroimaging [10]. A third review covers osteopontin, encoded by secreted phosphoprotein 1, a matricellular phosphoglycoprotein that interacts with microglial priming, vascular remodelling, myelin repair and innate immune responses during aging [11]. A randomized controlled trial in 120 patients with subacute spinal cord injury compared standard care alone with standard care plus a fixed-dose combination of inosine monophosphate, agmatine sulphate and L-carnosine (CGXNW) for 6 months, with neurological recovery and functional independence as endpoints [12].
