What markers track thyroid function?
A research-supply overview of the laboratory markers used to track thyroid function, drawn from registered trials and published studies in hypothyroidism, Hashimoto's thyroiditis and related endocrine models.
Thyroid function is tracked in the research literature through a panel of circulating markers rather than a single analyte. The conventional core is thyroid-stimulating hormone (TSH), free thyroxine (FT4) and free triiodothyronine (FT3), with thyroid peroxidase antibodies (TPO-Ab) and thyroglobulin antibodies (Tg-Ab) added where autoimmune aetiology is under investigation. These are peptide and glycoprotein hormones and immunoglobulins; the evidence block used here does not supply sequence, formula or molecular weight data for them, so none is stated.
The work described below centres on the hypothalamic-pituitary-thyroid axis and its interaction with peripheral metabolic, inflammatory and mineral-metabolism pathways, including the soluble alpha-Klotho-FGF23 axis and the adipokine system.

Vitamin E and oxidative stress markers in levothyroxine-treated hypothyroidism
A registered trial is examining whether vitamin E supplementation used as an adjunct to levothyroxine therapy reduces oxidative stress, improves antioxidant defence mechanisms and favourably influences thyroid hormone status in patients with hypothyroidism. The stated aim is to establish an evidence-based framework for integrating targeted antioxidant therapies into standard hypothyroid management [1]. The design positions oxidative stress markers alongside conventional thyroid hormone measurements as parallel readouts.
Adipokines and metabolic parameters with hormone replacement in newly diagnosed hypothyroidism
Primary hypothyroidism is associated with dyslipidaemia, insulin resistance, adipose tissue dysfunction and altered adipokine secretion. A prospective observational study is evaluating changes in serum asprosin, adipolin, omentin-1 and visfatin together with metabolic parameters in newly diagnosed primary hypothyroid patients before and after 8 weeks of levothyroxine replacement. The rationale is that biochemical restoration of thyroid hormone levels may not fully reflect metabolic improvement [8].
| Marker class | Examples named in the evidence | Context |
|---|---|---|
| Thyroid hormones | TSH, FT4, FT3 | Conventional panel [5] |
| Adipokines | Asprosin, adipolin, omentin-1, visfatin | Before and after 8 weeks levothyroxine [8] |
| Mineral metabolism | Soluble alpha-Klotho, FGF23 | Hashimoto's thyroiditis case-control [6] |
| Inflammatory | HMGB1 | Hashimoto's thyroiditis [12] |
Soluble alpha-Klotho and FGF23 in Hashimoto's thyroiditis
Hashimoto's thyroiditis is the most common autoimmune thyroid disease and is increasingly recognised as a condition involving chronic low-grade systemic inflammation beyond thyroid hormone dysfunction. A prospective case-control study is comparing serum soluble alpha-Klotho and FGF23 levels between adults with Hashimoto's thyroiditis and age- and sex-matched healthy controls, and investigating their associations with thyroid function parameters and inflammatory markers [6].
Omentin-1 and HMGB1 in Hashimoto thyroiditis
A separate study evaluated circulating levels of omentin-1, an anti-inflammatory adipokine, and high-mobility group box 1 (HMGB1), an inflammatory alarmin, in patients with Hashimoto thyroiditis, on the premise that the disease extends beyond abnormalities in thyroid hormone levels [12].
Thyroid function panel in a large obesity registry
A real-world registry study plans to enrol 10,000 patients with obesity across approximately 50 medical institutions. Baseline assessment includes a thyroid function panel of three items alongside complete blood count, urinalysis, liver and renal function, electrolytes, urine albumin/creatinine ratio, fasting glucose, fasting C-peptide, fasting insulin, HbA1c, blood lipids and hepatic elasticity ultrasound [5]. The panel is used here as one component of a broad metabolic characterisation rather than as a primary endpoint.
LH and hCG in adult endocrine assessment
Luteinizing hormone rises substantially after menopause and may also rise in some men; human chorionic gonadotropin is detectable in small amounts in some postmenopausal women and older men. A study is evaluating the role of both hormones in adult patients undergoing assessment for endocrine disorders, noting that recent research suggests they may influence bone health, body weight, kidney function, thyroid function and adrenal gland function, while little is established [7].
Thyroid function as a safety readout in an enclomiphene case report
A case report described a 46-year-old man with prior mitral valve repair and chronic immune thrombocytopenia in whom atrial flutter was first documented shortly after he inadvertently increased his enclomiphene dose from 25 mg to 50 mg daily. Laboratory evaluation showed chronic thrombocytopenia with a platelet count of 36 x 109/L, while thyroid function, cardiac biomarkers and electrolytes were normal [3].
Thyroid axis monitoring alongside long-acting growth hormone
An observational prospective monocentric study explored hormonal changes in children with isolated growth hormone deficiency (n = 33) starting the long-acting analogue somatrogon (n = 16) or switching from daily recombinant human growth hormone to the same weekly analogue (n = 17). The authors note that the impact of long-acting formulations on other pituitary axes remains unclear [3].
Thyroid function in pharmacogenomics-guided depression therapy
A prospective randomised parallel-controlled trial plans to enrol 220 patients with major depressive disorder, allocated 1:1 to pharmacogenomics-guided therapy or conventional treatment, with a 12-week follow-up and a matched healthy control cohort for cross-sectional biomarker comparisons [4]. Thyroid-related and other biomarkers are collected within a broader biomarker panel; the evidence block does not specify which thyroid analytes are measured.
Parathyroid function after lateral neck dissection
A retrospective analysis of 347 patients with papillary thyroid carcinoma compared outcomes across three progressively extensive lateral neck dissection schemes, examining the graded effect of dissection extent on parathyroid function [9].
Persistent TSH elevation despite levothyroxine in an adolescent
A case report and literature review described an adolescent with autoimmune thyroiditis in whom TSH remained elevated despite levothyroxine therapy, revealing autoimmune adrenal insufficiency. The authors note that routine screening for autoimmune adrenal insufficiency is not recommended in this population [10].
Goserelin versus leuprolide in premenopausal breast cancer
A retrospective cohort study compared goserelin and leuprolide, both GnRH agonists used for ovarian function suppression in premenopausal breast cancer patients receiving adjuvant endocrine therapy, asking whether their different molecular structures lead to clinically meaningful differences in efficacy and safety [11].
