What markers track tendon healing?
A research-supply overview of the markers and outcome measures used to track tendon healing, drawn from registered trials and preclinical tendon-repair studies.
Tendon healing is tracked through a combination of patient-reported functional scores, imaging-based structural assessment, and — in laboratory work — histopathological, histochemical, immunohistochemical and biomechanical endpoints. In the clinical trial literature, the Constant-Murley score (CMS) is used as a primary functional endpoint after rotator cuff repair, with follow-up extending to at least five years in at least one registered study [2]. In preclinical models, healing is tracked by tissue architecture on histopathology, collagen organisation on histochemistry, marker expression on immunohistochemistry, and load-to-failure or stiffness on biomechanical testing [6][7][8].
The mechanistic work in this area centres on growth factor and peptide signalling — including fibroblast growth factor, vascular endothelial growth factor, bone morphogenetic protein and thymosin beta-4 family signalling — and on macrophage polarisation as an upstream regulator of repair [5][6][7][8].
Research Studies
BPC-157 and tendon healing after arthroscopic rotator cuff repair in humans
A randomized, double-blind, placebo-controlled pilot trial is registered to assess BPC-157 (a synthetic peptide) for tendon healing and functional recovery following arthroscopic rotator cuff repair. Twenty patients are to be randomised to BPC-157 or saline placebo, self-administered subcutaneously once daily for 90 days after surgery, with all other care following the standard postoperative pathway. The registration notes that BPC-157 has shown regenerative and anti-inflammatory effects in preclinical animal models of musculoskeletal injury but has not previously been studied in formal human clinical trials [1].
Autologous microfragmented lipoaspirate and long-term Constant-Murley score after rotator cuff repair
A prospective randomized controlled single-blind trial evaluated autologous microfragmented lipoaspirate tissue injected intraoperatively during single-row arthroscopic rotator cuff repair. The primary hypothesis was that enzyme-free processing of the adipose tissue would improve clinical outcome measured by the Constant-Murley score at least five years after surgery [2].
Metformin and healing markers in chronic Achilles tendinopathy
An exploratory clinical trial in non-diabetic patients with chronic Achilles tendon pain is testing oral metformin against placebo, each combined with a home exercise programme. Twenty participants are to receive metformin capsules for 16 weeks and twenty are to receive placebo tablets. The stated aims are to assess effects on tendon pain and to determine effects on tendon healing and repair [3].
Sex hormone supplementation and tendon healing after shoulder repair in men with low hormone levels
A pilot randomized trial is enrolling men with low sex hormone levels who are undergoing shoulder tendon repair surgery. Participants are randomised to sex hormone therapy or placebo pills during healing, and the investigators are testing whether increased sex hormone levels show promise for tendon healing, shoulder function and pain [4].
bFGF and VEGFA gene delivery to tendon and peritendinous tissue in early healing
Preclinical work has addressed the limited vascularity and low cellularity of tendon by delivering basic fibroblast growth factor (bFGF) and vascular endothelial growth factor A (VEGFA) genes to tendon and the surrounding peritendinous tissues, which contain vascular, stromal and extrinsic reparative cells. The approach builds on earlier tendon-only co-delivery of the same factors and reports enhanced early tendon healing [5].
BPC-157, TB-500 and combination treatment in a rat Achilles tendon model
A rat study evaluated BPC-157, synthetic thymosin beta-4 (TB-500) and their combination in Achilles tendon healing using biomechanical, histopathological, histochemical and immunohistochemical analyses. Thirty-two male Sprague-Dawley rats were used [6].
BMP-7 and low-molecular-weight peptide solution in a rat rotator cuff tear model
Because rotator cuff tears are the most prevalent shoulder pathology requiring surgery and retear rates remain high, a rat study assessed bone morphogenetic protein-7 (BMP-7) and a low-molecular-weight peptide solution (LMWPS) as biological adjuncts for early tendon-to-bone healing, using histopathological and biomechanical endpoints [7].
GHRP-2, M1 macrophage polarisation and tendon-bone healing in a rat rotator cuff model
Growth hormone-releasing peptide 2 (GHRP-2) was explored for tendon-bone healing in a rat rotator cuff tear model. The effect on M1 macrophage polarisation was first determined in vitro by real-time polymerase chain reaction, Western blot and immunofluorescence, with histologic and biomechanical healing properties then assessed in the model [8].
