NAD+ vs BPC-157 + TB-500 blend: what the research compares

A comparison of what published research has measured for NAD+ and for a BPC-157 + TB-500 blend, where the evidence is stronger for one, and what remains unknown.

NAD+ (nicotinamide adenine dinucleotide) is a dinucleotide coenzyme with the formula C21H27N7O14P2 and a molecular weight of approximately 663.43 g/mol. It is also referred to in the literature as nicotinamide adenine dinucleotide, and its reduced form is NADH. The BPC-157 + TB-500 blend is a research combination of two synthetic peptides: BPC-157 (a pentadecapeptide with the sequence Gly-Glu-Pro-Pro-Pro-Gly-Lys-Pro-Ala-Asp-Asp-Ala-Gly-Leu-Val, molecular weight approximately 1419.53 g/mol) and TB-500 (a synthetic fragment of thymosin beta-4, also known as thymosin beta-4 fragment, with the sequence Leu-Lys-Lys-Thr-Glu-Thr-Gln, molecular weight approximately 889.0 g/mol). Both are supplied as research-use-only materials.

The published work on NAD+ centres on redox metabolism and the sirtuin and PARP enzyme families, while the published work on the BPC-157 + TB-500 blend centres on growth factor signalling, cell migration, and actin cytoskeleton regulation.

Research Studies

NAD+ and retinal protection in primary open-angle glaucoma

A clinical trial is investigating whether high-dose nicotinamide, a precursor of NAD+, can protect retinal nerve cells in adults with primary open-angle glaucoma who are already receiving standard glaucoma eye drops or laser treatment. The trial measures the rate of thinning of the inner retinal nerve cell layer over 12 months using a non-invasive eye scan, comparing participants randomised to nicotinamide plus standard therapy against those receiving standard therapy alone. Participants in the treatment group take nicotinamide tablets by mouth, starting at 1.5 grams per day [1].

NAD+ and post-COVID syndrome with tryptophan deficiency

An observational study is examining a special nicotinamide formulation (CICR-NAM) as a food for special medicinal purposes in patients with post-COVID syndrome. The rationale notes that changes in tryptophan metabolism, energy metabolism, and the gut microbiome may occur in this population, and that in an earlier nutritional study patients with COVID-19 recovered physical performance more quickly when taking nicotinamide. Patients who responded to nicotinamide also developed post-COVID syndrome less frequently. The CICR-NAM tablets were developed to release the active substance selectively in the lower intestine and have been shown to be well tolerated in several studies. Patients with post-COVID syndrome and documented tryptophan deficiency may take the tablets [2].

BPC-157 + TB-500 blend and perineural injection models

No published study in the supplied evidence directly examines the BPC-157 + TB-500 blend. The closest related clinical research compares perineural 5% dextrose injections and perineural local anesthetic injections in patients with symptomatic knee osteoarthritis. That prospective observational study follows adult patients receiving routine perineural injection therapy for six months, assessing pain intensity and functional outcomes at baseline and at 1, 3, and 6 months using validated clinical assessment tools [3]. This study does not involve BPC-157 or TB-500 and is included only to illustrate the type of perineural injection research that exists.

NAD+ and cardiovascular outcomes in atherosclerotic disease

A trial is evaluating maridebart cafraglutide, not NAD+, for cardiovascular outcomes in participants with atherosclerotic cardiovascular disease and overweight or obesity. The primary objective is to demonstrate superiority over placebo as an adjunct to standard of care in reducing cardiovascular morbidity and mortality [4]. This is not an NAD+ study and is noted here only to distinguish the metabolic cardiovascular research space from NAD+ research.

BPC-157 + TB-500 blend and Crohn's disease induction therapy

A Phase III, multicenter, double-blind, placebo-controlled study is assessing induction therapy with afimkibart (RO7790121) in participants with moderately to severely active Crohn's disease [5]. This study does not involve BPC-157 or TB-500. It is referenced to show that the inflammatory bowel disease research landscape uses biologic agents rather than the peptides in the blend.

NAD+ and wound debridement

A post-market clinical follow-up study is comparing Debrisoft Duo with Debrisoft Pad in the debridement of wounds, confirming performance and collecting safety data within certified indications under routine use [6]. This study does not involve NAD+ or the BPC-157 + TB-500 blend. It is included to illustrate the wound care research context in which peptide-based wound healing claims are sometimes placed.

BPC-157 + TB-500 blend and fibromyalgia posture

A study is examining forward head posture frequency in fibromyalgia patients assessed by craniovertebral angle and its association with symptom severity. Forward head posture is reported to be more common in patients with fibromyalgia and is a factor that increases symptom severity [7]. This study does not involve BPC-157 or TB-500. It is referenced to show the musculoskeletal and pain research space that is sometimes conflated with peptide research.

NAD+ and heart attack outcomes

A research study is looking at how ziltivekimab works compared to placebo in people with a heart attack. The study is being done to see if ziltivekimab can be used to treat people admitted to hospital because of a heart attack, and whether it might reduce development of heart disease, thereby preventing new heart attacks or strokes. Participants receive either ziltivekimab or placebo by chance, injected once every month into a flat skin surface in the stomach, thigh, or upper arm. Ziltivekimab is not yet approved in any country or region [8]. This is not an NAD+ study and is included only to distinguish the cardiovascular inflammation research space from NAD+ research.

NAD+ and myocardial fibrosis

A reference-only paper reports that Guizhi Wuling Decoction alleviates myocardial fibrosis by restoring mitochondrial homeostasis, based on network pharmacology, molecular docking, and multi-omics integration. The decoction is a combined prescription consisting of the classical formulas Wuling San and Guizhi Fuling Wan without modification, and has been clinically applied for cardiovascular diseases. Previous studies have reported its cardioprotective and anti-fibrotic effects [9]. This paper does not involve NAD+ or the BPC-157 + TB-500 blend and is included as supporting context for mitochondrial and fibrosis research.

Where the evidence is stronger for one

The supplied evidence contains direct clinical research on NAD+ in two populations: adults with primary open-angle glaucoma receiving standard therapy [1], and patients with post-COVID syndrome and documented tryptophan deficiency [2]. For the BPC-157 + TB-500 blend, the supplied evidence contains no direct study. The only related clinical research is a perineural injection study in knee osteoarthritis that does not involve either peptide [3]. On the basis of the supplied evidence alone, the published work is stronger for NAD+ because it includes named populations and measured outcomes, while the blend has no direct clinical data in the supplied evidence.

What is simply not known

For NAD+, it is not known whether adding nicotinamide to standard glaucoma therapy slows the rate of thinning of the inner retinal nerve cell layer over 12 months [1]. It is not known whether CICR-NAM affects tryptophan metabolism, energy metabolism, the gut microbiome, or physical performance in post-COVID syndrome [2]. For the BPC-157 + TB-500 blend, the supplied evidence does not establish whether the blend has any effect in any human population, what dose might be relevant, or what safety profile it has. No study in the supplied evidence measures the blend directly.
