How does intestinal permeability work?

Intestinal permeability describes the regulated passage of solutes across the gut epithelium. This page covers the paracellular and transcellular routes, the tight-junction machinery that gates them, and what laboratory

Intestinal permeability is the regulated passage of water, ions, nutrients, microbial products and larger solutes across the intestinal epithelium, the single-cell layer that separates the gut lumen from the underlying lamina propria and systemic circulation. It is not a single channel but a set of parallel routes: a transcellular route through the apical and basolateral membranes of enterocytes, and a paracellular route through the intercellular space sealed by tight junctions. Synonyms in the literature include gut barrier function, epithelial barrier integrity, and intestinal barrier permeability. The term is a functional descriptor rather than a molecular identifier — it has no sequence, formula or molecular weight, because it names a property of a tissue rather than a compound. In practice it is quantified by the flux of probe molecules across the epithelium, most commonly the urinary lactulose : mannitol ratio collected over a defined window after an oral dose [6].

The molecular work in this area centres on the tight-junction protein family — claudins, occludin and the zonula occludens scaffold — together with the mucosal immune and microbial signals that regulate their assembly and disassembly. Because the tight junction is the rate-limiting structure for the paracellular route, most mechanistic studies of intestinal permeability are, in effect, studies of tight-junction regulation.

Red fish scale collagen peptides and barrier integrity in Caco-2 cells and murine colitis

A low-molecular-weight collagen peptide fraction (LSCP-I, under 3 kDa) derived from Lutjanus erythropterus scales was evaluated in Caco-2 cell monolayers subjected to macrophage-mediated inflammatory injury and in murine models of colitis. In the cell model, LSCP-I at 50 µg/mL increased proliferation by 35% and migration by 40%, preserved barrier integrity, reduced reactive oxygen species by 45%, decreased lipid peroxidation by 30%, and restored glutathione and superoxide dismutase [1]. The design separates an in vitro epithelial repair readout from an in vivo colitis readout; the barrier-preservation finding in Caco-2 cells is an in vitro observation, and the murine colitis component is animal-only.

Oral glutamine and sex-dependent gut responses in diet-induced obesity

Male and female C57BL/6 mice received a standard diet or a high-fat diet (60% kcal from fat) for 14 weeks; from week 12, animals received glutamine in drinking water at 2 g/kg/day or no supplementation. The study characterised body composition, glucose tolerance, insulin sensitivity, intestinal permeability and colon outcomes, and reported opposite metabolic and gut responses between male and female mice [2]. This is an animal study, and the sex divergence is the central finding — permeability outcomes cannot be assumed to be uniform across sexes on the basis of this work.

Zein-based nanoparticles and oral peptide permeability in Danio rerio

Hybrid zein-Eudragit RS100-chitosan nanoparticles were developed for oral co-delivery of insulin and liraglutide, addressing enzymatic degradation and poor intestinal permeability as the two principal barriers to oral peptide administration. The particles were prepared by nanoprecipitation and showed nanoscale size (~230 nm), low polydispersity (PDI < 0.1) and positive zeta potential (~+46 mV), with structural and thermal analysis indicating a predominantly amorphous and molecularly dispersed payload [3]. Preliminary in vivo evaluation was performed in Danio rerio; this is a non-mammalian model and the permeability claim is about formulation behaviour, not about human absorption.

Collagen peptide dose and aspirin-challenged permeability in healthy adults

A registered trial evaluates 45 days of daily collagen peptide at three doses (2.5 g, 5 g and 10 g) versus placebo in healthy adults, with intestinal permeability measured by the urinary lactulose : mannitol ratio over a 5-hour collection after an acute aspirin challenge. The primary comparison is the change from Day 42 to Day 45 between each dose and placebo [6]. The aspirin challenge is used to induce a transient permeability increase against which a protective effect can be detected.

Fiber-enriched diet and permeability in alcohol use disorder

A clinical trial in patients with alcohol use disorder aged 19–65 tests whether a fiber-enriched diet high in inulin and pectin, combined with standard care, reduces intestinal permeability. The stated hypothesis is that fiber modifies the gut microbiota and its metabolites, decreasing permeability and restoring microbiota composition, with further effects on abstinence, craving, anxiety, inflammation, steatosis and hepatic fibrosis [7]. This is a dietary-intervention design; the permeability endpoint is one of several.

Sugar-based synbiotic and intestinal health in overweight adults

A randomized, double-blind, controlled crossover study in healthy overweight individuals compares a sugar-based synbiotic containing inulin and Bacillus coagulans BC30 spores against a matched control sugar product containing inulin without spores. Twenty participants complete a 2-week run-in and two 4-week intervention periods separated by a 4-week washout, consuming two sachets daily; the active arm provides 1 × 10⁹ CFU/day. The primary outcome is change in fasting markers of subclinical inflammation, including high-sensitivity C-reactive protein [5]. Intestinal health is assessed alongside cardiometabolic markers rather than as the sole endpoint.

Akkermansia muciniphila, berberine and barrier integrity in night-shift workers

Night shift work is associated with increased risk of obesity, insulin resistance and cardiometabolic disorders, attributed in part to circadian misalignment, disrupted sleep and altered eating patterns, with gut microbiota as a modifying factor. Akkermansia muciniphila has been linked to improved metabolic health, including enhanced insulin sensitivity, lipid regulation and maintenance of intestinal barrier integrity, and berberine has been reported to upregulate A. muciniphila and improve insulin sensitivity. A registered trial examines supplementation with both in night-shift workers [8]. The barrier-integrity link here is a rationale drawn from prior literature, not an outcome of this trial.

Exercise and integrated permeability responses in elite rowers

An observational study of thirty members of the Polish Youth National Rowing Team, aged 19–24, examines multilevel physiological, molecular, metabolic, intestinal, immunological and psychophysiological responses to rowing-specific exercise across two training periods, including a 2000-m maximal ergometer effort during the competitive phase. Intestinal permeability is one of the integrated adaptive mechanisms under investigation, alongside mitochondrial function, metabolic regulation, immune activation and DNA damage response [4]. This is an observational design in a trained athletic population.

Permeation enhancers and epithelial transport of a PROTAC in rats

Proteolysis targeting chimeras frequently fall outside rule-of-five chemical space, which is associated with poor intestinal solubility, permeability and absorption. Work in rats examined how intestinal permeation enhancers affect solubilisation and epithelial transport of the PROTAC ARV-471, noting that formulation efforts have focused mainly on solubility while intrinsic permeability remains a limiting factor [9]. This is a rat study.

Natural products and probiotics in NSAID-induced small intestinal injury

A systematic review evaluated natural products, functional foods and probiotics for preventing or treating NSAID-induced small intestinal injury, a clinically significant complication among chronic NSAID and aspirin users for which effective treatment options remain limited, with mechanistic consideration given to oxidative stress and microbiome modulation [10]. Related work examined the gastroprotective potential of canagliflozin in indomethacin-induced peptic ulcer, with attention to inflammation, pyroptosis, autophagy and gut integrity [12]. Both are relevant to permeability because NSAID injury disrupts the epithelial barrier.

Permeability in metabolic and endocrine contexts

A review of diagnostic and prognostic biomarkers in polycystic ovary syndrome examines insulin resistance as a central molecular driver of metabolic and reproductive complications, including infertility, obesity and increased long-term cardiovascular risk [11]. Insulin resistance and barrier function are frequently studied together because both are tied to metabolic and inflammatory signalling.
