How does exosomes work?

Exosomes are extracellular vesicles that transfer proteins, lipids, and nucleic acids between cells. This page outlines the mechanisms reported in laboratory and clinical research, including the pathways and models studi

Exosomes are a class of extracellular vesicles, typically 30–150 nm in diameter, released by cells upon fusion of multivesicular bodies with the plasma membrane. They carry proteins, lipids, and nucleic acids and act as intercellular messengers. The studies cited here center on exosome-mediated transfer of cargo and its downstream effects, often involving receptor tyrosine kinase signaling, such as EGFR, and metabolic pathways like AMPK/mTOR.

Research into exosome function spans in vitro assays, animal models, and human clinical studies. The evidence below is drawn from these sources and is presented for laboratory research context only.

Research Studies

Exosomal hsa-miR-221-5p and keratinocyte LL-37 in melanoma progression

In melanoma, tumor-derived exosomes were found to suppress the expression of cathelicidin LL-37 in keratinocytes. This suppression was driven in part by exosomal hsa-miR-221-5p through impaired sustained EGFR signaling. Functionally, keratinocyte-derived LL-37 curtailed melanoma cell migration, invasion, and epithelial-mesenchymal transition, restrained tumor growth in vivo, and was associated with a tumor immune microenvironment enriched in M1 macrophages [2].

FGF21-modified adipose stem cell-derived exosomes and fibroblast glycolysis in wound healing

Adipose-derived mesenchymal stem cells engineered to overexpress FGF21 released exosomes enriched with FGF21 (Exo@FGF21). These exosomes promoted wound healing by activating fibroblast glycolysis through AMPK/mTOR signaling. The study characterized the exosomes using transmission electron microscopy and nanoparticle tracking analysis [3].

Cerebrolysin-treated endothelial cell exosomes and endothelial permeability

Exosomes released by Cerebrolysin-treated cerebral endothelial cells (Cerebro-Exos) showed marked differences in cargo profiles compared to exosomes from untreated cells (Naïve-Exos). Cerebro-Exos were highly enriched with metabolic and tight junction-related proteins. In an in vitro endothelial permeability assay, Cerebro-Exos had a superior effect compared to Naïve-Exos on restoring endothelial cell integrity impaired by fibrin and tissue plasminogen activator (tPA) [4].

Urinary exosome podocyte markers in diabetic kidney disease

A clinical study is investigating podocyte-specific injury markers (nephrin, podocalyxin, and Wilms' tumor protein WT-1) in urinary exosomes from African American Veterans with albuminuric stage 2 and 3 chronic diabetic kidney disease. The study involves treatment with empagliflozin, finerenone, or combination therapy. Secondary objectives include correlating exosome-based podocyte-specific proteins with standardized biomarkers of kidney injury (urine albumin/creatinine ratio and estimated GFR) and with systemic inflammatory markers such as interleukins (IL-1, IL-6, IL-12), hs-CRP, and arterial stiffness measurements [1].

Neuronal exosomes and exercise-induced anxiolysis

A clinical study is examining the brain-based biological mechanistic pathway by which exercise exerts anxiolytic effects. The investigators will measure brain-derived markers of mitochondrial metabolism (acetyl-L-carnitine) and inflammation (interleukin-6) using technology to isolate neuronal exosomes [7].

Wharton's jelly exosomes with bone marrow cells in liver cirrhosis

A clinical study is evaluating the use of autologous bone marrow-derived mononuclear cells combined with Wharton's jelly exosomes (20 billion per dose) administered intravenously in patients with liver cirrhosis. The protocol excludes individuals with active tumors or other major health issues [8].

Umbilical cord blood-derived stem cells and exosomes in autism spectrum disorder

A 12-month clinical study is testing whether AdiaVita (umbilical cord blood-derived stem cells and exosomes) plus glutathione improves autism symptoms more than glutathione alone in children ages 3–12. Approximately 100 children with confirmed autism will be randomly assigned to receive either monthly IV AdiaVita plus IV glutathione for 3 months or IV glutathione plus a saline placebo infusion for 2 months. Both groups also use a topical glutathione spray twice daily at home. Symptoms are tracked with the Autism Treatment Evaluation Checklist (ATEC), completed by a parent and by a therapist or teacher. Safety is monitored through Month 12 [5].

Regenerative medicine approaches to craniofacial and corneal neuropathic pain

A review discusses regenerative medicine approaches for craniofacial and corneal neuropathic pain, conditions characterized by persistent pain refractory to conventional therapies. These disorders involve peripheral nerve injury, neuroinflammation, and central sensitization within trigeminal pathways. The review emphasizes mechanism-based, patient-stratified treatment strategies and a shift from symptom suppression toward structural nerve repair and functional restoration [6].
