What is autophagy?
The cell's own recycling programme, why it declines with age, and which registered studies are trying to measure and switch it on.
Autophagy — literally "self-eating" — is the process by which a cell encloses damaged proteins and worn-out organelles in a membrane, delivers them to the lysosome and breaks them down for reuse. It is a housekeeping pathway present in every nucleated cell, and its decline with age is one of the mechanisms the ageing literature returns to most often [2].
The pathway family is nutrient sensing: autophagy is suppressed when nutrients are abundant and released when they are scarce, which is why fasting and exercise are the interventions most studied for activating it [1].
Research Studies
Autophagy and the therapeutic strategies for ageing
A 2026 review of therapeutic strategies for ageing and age-associated disease places autophagy among the core targets, alongside senescence and mitochondrial decline [2]. It is a review: it collects what has been proposed and reports where the evidence is preliminary.
Autophagy and fasting, exercise and diet
A registered trial is testing fasting, exercise and diet as a combined intervention to activate autophagy in the context of depression [1]. It is registered, not reported — the register gives the design and the outcome measures, not the results.
Autophagy and supplementation biomarkers
Another registered study is measuring biomarkers of healthy ageing and autophagy under a supplement intervention [6]. Its interest for a reader is the outcome set: which markers a trial considers evidence that autophagy has changed.
Autophagy and hepatic fibrosis
A review of pharmacological interventions for hepatic fibrosis discusses autophagy among the pathogenic mechanisms being targeted [5] — an example of the pathway appearing as a mechanism inside a disease literature rather than as a subject on its own.
Autophagy and skeletal muscle
A 2026 study of myostatin overexpression in skeletal muscle reports effects on oxidative capacity and touches the autophagy machinery in that tissue [3]. Mouse model; the finding is about muscle biology rather than about an intervention.
Autophagy and nutrient sensing
Two kinases sit at the switch. When amino acids and growth signals are plentiful, mTOR is active and suppresses the initiation of autophagy; when energy runs low, AMPK is active and promotes it. Fasting, caloric restriction and exercise are studied as activators precisely because they shift that balance [1], and the ageing reviews treat the gradual loss of that responsiveness as one of the reasons damaged material accumulates in older cells [2]. The pathway is therefore not "on" or "off" but tuned, and interventions are attempts to retune it rather than to start something that has stopped.
Autophagy and how it is measured
Autophagy is inferred, not watched: flux assays, marker proteins such as LC3 and p62, and electron microscopy of autophagosomes. Trials in this pool report "biomarkers of autophagy" as an outcome without a single agreed standard, which is a limitation the field itself acknowledges.
Autophagy and the compounds studied alongside it
The peptide literature intersects with autophagy mostly through mitochondrial and nutrient-sensing work. Those crossings have their own pages in this journal; this one is the hub they return to.
